2025 Open Enrollment

This is the one time a year you can enroll in or make changes to your benefits.

Open enrollment is your opportunity to enroll into or make changes to your benefits for the new plan year, effective January 1, 2025 to December 31, 2025. Benefits selected will remain in effect the whole plan year, unless you experience a qualifying life event.

Important Action Items to Remember During the Open Enrollment Period:

  • Please review your benefits information below carefully so you can make an informed decision and choose the plans that are best for you (and your family) this upcoming plan year.
  • Prime healthcare will only be verifying newly enrolled dependents who will be added in the plan for the 2025 year. Current enrolled dependents will not need to be reverified.
    • You must present the appropriate documentation to verify your dependent. The list of documentation needed is found under the Dependent eligibility chart in your Benefits guide.
    • To enroll dependents, you will need to make a Virtual or Face to Face appointment with a Benefits Counselor by calling 866-998-2915 or online at www.myenrollmentschedule.com/prime.

 

The following benefits do not roll over. If you are enrolled and would like to continue coverage into 2025, you must re-elect:

FSA – Medical or Dependent Care

MERP – MERP Information and Eligibility requirements can be found in your Benefits guide. If selecting MERP make sure you have all necessary information regarding your current medical carrier (Carrier name, ID Number, and Copy of the Insurance Card). The MERP Attestation needs to be completed in Lawson.

Medical Waiver – If choosing to waive Prime Healthcare coverage due to other medical coverage, make sure you have all necessary information regarding your current medical carrier (Carrier name, ID Number, and Copy of the Insurance Card). Please fill out the Medical Waiver form and submit it to your HR/Benefits coordinator before 11/29/2024

Opt-Out Credit – If you would like to request an Opt-out credit due to other medical coverage, make sure you have all necessary information regarding your current medical carrier (Carrier name, ID Number, and Copy of the Insurance Card), fill the Opt-out form and submit it to your HR/Benefits coordinator before 11/29/2024

This is also a good time to make sure your personal information is updated in Lawson. Please log on with your User ID and Password – the same one you use to log into your workstation.

Social Security Identification: – In order to meet IRS requirements mandated by the Affordable Care Act (ACA), the enrollee’s name and social security number entered into the enrollment system, must match EXACTLY as stated on the Social Security Identification card including hyphenated names.
